If you would like to register as a patient, please download and complete the two forms below; both the practice medical questionnaire and health board form. These can also be obtained from reception. Completed forms should be returned to the practice, preferably in person, with the required identification, as specified in the registration form.
Acceptance on to the list is as at the discretion of the Doctor. We may also ask you for proof of identity. (Please refer to our registration policy below and why we ask for this).
Your medical records may take several weeks to arrive at the Practice.
There are a number of reasons why you may not be able to register with your chosen GP. For example, you may live too far away. If this is the case simply choose another GP in your local area.

We can accept patients who live in New Cumnock, Logan, Lugar and other surrounding areas. Please contact the Practice if you wish to register and are unsure if you are within our area and we can advise you.
